Electronic Visit Verification (EVV) is required for all in-home visits. LIVEWELL uses WellSky for EVV.
• Always clock in at the client's location — GPS verification is required.
• Never clock in before arriving or clock out after leaving.
• If EVV isn't working, call the office for a manual entry.
This course covers essential knowledge and skills for providing compassionate care to clients living with dementia and Alzheimer's disease.

Personal Care — 16 hours/week (Mon, Wed, Thu, Fri · 2:00–6:00 PM). Authorization valid through September 30, 2026.
Dorothy has mild hearing loss in her left ear. Speak clearly and face her when talking. She is allergic to penicillin. She uses a cane for longer walks but moves independently around the house. Monitor for signs of dizziness or unsteadiness.
Keep front door locked at all times per family request. Dorothy may occasionally forget and try to go outside unaccompanied — gently redirect.
Prefers to be called "Dot." Enjoys watching Jeopardy at 5 PM — try to have dinner prep done before then. Likes her tea with one sugar, no milk. Enjoys conversation about her grandchildren and gardening.
Log all visit notes in the app after each shift. Report any falls, health changes, or behavioral concerns immediately to your coordinator Jessica Thompson. Daughter Sarah Mitchell should be contacted for non-urgent family matters.

Provide meaningful social engagement and companionship for James. Support his wife Margaret with respite time. Encourage light physical activity and mental stimulation.
Companion Care — 6 hours/week (Tue, Fri · 9:00 AM–12:00 PM). Authorization valid through August 31, 2026.
James uses a walker at all times. Ensure pathways are clear of tripping hazards. He has mild cognitive decline — may repeat stories or forget recent events. Be patient and do not correct him. Monitor for confusion or agitation and report any changes.
James is a retired high school history teacher and loves to share stories about his career. He drinks black coffee in the morning. He prefers to sit in his recliner by the window. Do not rearrange items on his desk — he has a specific system.
Margaret is usually home during visits but uses the time for errands or rest. Check in with her briefly at arrival and departure. Log notes after each visit. Report concerns to coordinator Jessica Thompson.

Support Ruth in maintaining a clean, safe, and comfortable home environment. She is independent with personal care and values her autonomy — homemaker services focus on tasks that have become physically difficult for her.
Homemaker — 8 hours/week (Wed, Sat · 10:00 AM–2:00 PM). Authorization valid through July 31, 2026.
Ruth has arthritis in both hands and mild back pain, which limits her ability to do heavy cleaning. She manages her own medications and does not need reminders. She is alert and fully oriented. Has a small dog named Biscuit — friendly but may bark at first.
Ruth has strong preferences about how her home is organized. Always ask before rearranging anything. She enjoys Korean cooking and may teach you recipes. She watches K-dramas in the afternoon — try not to vacuum during that time. She prefers eco-friendly cleaning products (under the kitchen sink).
Ruth communicates directly and will tell you what she needs. Son David Kim is the emergency contact but Ruth prefers to handle things herself when possible. Log notes after each visit. Report any home safety concerns to coordinator.
